Participant Assistance and Care

Participant Assistance and Care allows a person with a disability to remain and live successfully in their own home and function and participate in their communities.  PAC services support and enable the person in daily living and self-care activities.

Bridges of Indiana can provide this service to your loved one in your home or in your community.  This is a goal driven service that will allow our team of Direct Support professionals to work closely with a person to achieve a set of defined person-centered outcomes as defined by the individual’s care team.  Goal achievement is monitored quarterly and changed if necessary.
